Study finds treatment that may help older people with dry eyes due to loss of sex steroid hormones

A new study led by researchers at the University of Kyoto has reported that the loss of sex steroid hormones produced by the enzyme 3-hydroxyl-steroid dehydrogenase, or 3-HSD, causes the condition, but can treat it with nicotinamidemononucleotide, or NMN, which is a non-steroidal agent.

“Meibomian glands are found in the eyelids and play a role in the production of oil that closes the eyes. Without this oil, tears flow,” said project manager Masao Doi.

Despite the high incidence of meibomian virus, it does not appear to be life-threatening. However, unhealthy conditions can cause serious eye damage if left untreated.

There is currently no cure for meibomian disease because the mechanisms underlying it are still unknown.

As we get older, our own hormone production it is declining. The most well-known examples are sex hormones, such as testosterone and estrogen, which are clearly related to age-related factors of meibomian disease.

Normally, hormones are produced by the endocrine organs and then enter the bloodstream where they travel to regulate other parts of the body. However, using mutant rats, studies have shown that instead of this endocrine system, the meibomian gland relies on a recently discovered intracrine system in which the hormone acts in the tissue where it is synthesized.

The main interest of the 3-HSD group lies in its circadian rhythm, which is responsible for hundreds of motor behaviors such as sleep, menstruation, and secretory hormones. Further research on these circadian rhythms is expected to provide additional clues on the management of 3-HSD condition.

Doi added, “We have found that the enzymatic activity of 3-HSD peaks in mice as they sleep. This observation indicates when drug administration is important.”

The oscillating activity of 3-HSD is highly correlated with the 3-HSD cofactor nicotinamide adenine dinucleotide, or NAD, which is a coenzyme found in all cells and is marketed as an over-the-counter supplement. It also receives attention as a compound in drug synthesis for its ability to activate or inhibit hormone-producing enzymes including 3-HSD.

Confirming this point, studies show that the use of nicotinamide mononucleotide, or NMN, a precursor of NAD, increases the amount of NAD required to reactivate 3-HSD functions. Periodic addition of NMN to the meibomian gland of rat mice reduces atrophy of the meibomian gland, effectively reducing dryness in the eyes.

Although steroids, such as testosterone, usually have unintended side effects, Doi said, “Our research shows that it is possible to avoid this problem not only with home remedies but also with an unhealthy agent, such as su NMN. ”
